SMS PATIENT SURVEYS
AUTOMATED, HIPAA-COMPLIANT SMS PATIENT SURVEYS, BUILT FOR HEALTHCARE
Email surveys go unopened. Phone surveys burn staff hours. And by the time feedback trickles in, unhappy patients have already found your Google page. Dialog Health puts your survey where patients actually look - their texts - and brings answers back in minutes, not weeks.

Automatic SMS fallback, so every patient gets the message

Trusted by leading healthcare organizations.
YOU BUILT A GREAT BENEFITS PACKAGE. NOW YOU HAVE TO GET EMPLOYEES TO ACT ON IT.
The average HCAHPS response rate hovers around 23% - meaning four in five patients never tell you how their visit went. Email invites get buried. Phone and mail cost a fortune and reach fewer people every year. Meanwhile, the unhappy patients who do have something to say skip the survey and post a one-star review instead. Texting flips the script: the survey lands on the one screen patients always check, and feedback comes back while the visit is still fresh enough to act on.

A national contact center needed patient feedback it could actually act on - to predict retention, refine home health programs, and reach dissatisfied patients before they walked away. Using Dialog Health's HIPAA-compliant two-way texting, the team sent an automated, conversational post-visit survey to 10,261 patients. The results beat every expectation.

FREQUENTLY ASKED QUESTIONS
How do SMS patient surveys get higher response rates than email?
Texts are opened around 98% of the time, most within minutes, versus roughly 20–30% for email. Patients answer in the app they use all day, so the survey never sits unopened. In a March 2025 rater8 study of nearly 700 practices, moving from email to text lifted response rates from 22.40% to 37.52%. In our own contact center case study, 52% of patients replied.
Is texting patient surveys HIPAA-compliant?
Yes, when it's done correctly - and Dialog Health is built for it. From hosting and storage through processing and transmission, the platform adheres to HIPAA, TCPA, CTIA, SSAE, and SOC II standards. The best practice is to keep questions PHI-light: the text points to a secure form, and any sensitive detail stays behind the link, never in the message thread.
What about TCPA, consent, and opt-outs?
Communications for treatment, payment, or healthcare operations generally require prior express consent - typically satisfied when a patient shares their mobile number for that relationship. Every campaign includes clear opt-out language such as "Reply STOP." Under the FCC's consent-revocation rule (adopted February 2024, effective April 11, 2025), patients can revoke consent by any reasonable method - words like "stop," "quit," "cancel," or "unsubscribe" all count - and Dialog Health tracks opt-ins and opt-outs at the patient level.
Can Dialog Health replace our official HCAHPS or CAHPS survey?
No - and we won't claim it can. Official CMS CAHPS surveys use prescribed vendors and modes (including new web-first modes as of January 2025). Dialog Health SMS surveys are pulse surveys that supplement CAHPS: they capture fast, frequent feedback between official surveys so you can catch and fix issues early - and improve the experience that drives your official scores.
How soon after a visit should we send a survey?
Best practice is within 24–48 hours of an outpatient visit and within 72 hours of a hospital discharge, while the experience is still fresh. Dialog Health automates the timing so surveys trigger after discharge or checkout without any staff action.
How long should an SMS survey be?
Short wins. One to three questions earn the highest completion rates; keep full forms to roughly 5–10 questions. Our two-way format lets you open with a single rating and branch into follow-ups only when needed, so patients are never buried in questions.
Can we build our own survey forms?
Yes. Digital Forms lets your team build custom survey forms in-house - the same builder used for intake and registration. Text a branded link, the patient completes the form on their phone, and responses flow to your dashboard. One platform, one vendor, no separate survey subscription.
Can we route happy patients to leave a Google review?
You can invite patients to share their experience on Google or Facebook right in the survey flow, exactly as our case study shows. One important caveat: Google and the FTC prohibit "review gating" - deciding who gets the public review link based on how satisfied a patient is. The compliant best practice is to invite all patients to review and to flag low scores for private service recovery.
How do we handle unhappy patients?
This is where two-way texting shines. When a patient gives a low score, the survey automatically asks what went wrong, and AnalyticsPRO flags the response so your team can follow up fast - closing the loop and recovering the relationship before a bad experience becomes a public review.
Will surveys reach non-English-speaking patients?
Yes. The AI Translator supports 100+ languages with healthcare-aware translation. Staff compose in English and the system translates inbound and outbound messages automatically - with no app for the patient to download. One client, St. Louis Integrated Health Network, improved its response rate by 380% within 60 days of adding multi-language messaging.
Does it integrate with our EHR and other systems?
Yes. Dialog Health is integration-agnostic and built to enhance the technology you already use - including Epic, Cerner, Greenway, ModMed, NextGen, Meditech, and Athenahealth - via CSV, HL7, XML, or JSON over UI, sFTP, or Web API. That means surveys can trigger automatically off real events like discharge or checkout.
What about deliverability and 10DLC?
Since February 2025, carriers block unregistered A2P 10DLC traffic outright. Dialog Health is a Tier-1 texting provider with short codes and 10DLC, so your surveys are properly registered and delivered reliably - the reason our case study reached a 94% reach rate across 10,261 patients.
How do we avoid survey fatigue?
Keep surveys short, relevant, and capped - a common best practice is no more than once per 90 days per patient. Dialog Health's smart targeting, personalization tags, and automation let you throttle frequency and tailor questions by visit type, so every survey feels worth answering.
What analytics and reporting do we get?
AnalyticsPRO provides real-time delivery receipts, click tracking, reply and opt-out data, form-submission analytics, and campaign A/B comparison - all exportable. You can see who received, opened, and completed each survey, and exactly where to focus follow-up.
Are the survey links secure and trustworthy?
Yes. DH Links, our proprietary short-link technology, lets you brand and shorten every URL and track who clicked. Branded links look trustworthy, get clicked more often, and route patients to a secure, mobile-optimized form.
What results should we expect?
Results vary by organization and usage, but Dialog Health clients have reported an 83% survey response rate and an 11% increase in overall HCAHPS score, and one hospital surgical department raised patient satisfaction from 83% to 100% during a 90-day pilot. Our featured contact center case study saw a 52% reply rate and 84% satisfaction across 10,261 patients.
How fast can we launch, and what does it cost?
Fast. Dialog Health is cloud-based with a no-code, self-service console and best-practice healthcare templates, backed by a client success team that guides go-live. Pricing is tailored to your organization's size and use case. The best next step is a quick, no-pressure demo.
